Consultation Services

About this Program

Consultation Services is a participant's first step in beginning the Community First Services and Supports (CFSS) program. Participants will meet with their designated MRCI representative to develop a person-centered Service Delivery Plan.

 

The purpose of Consultation Services is to educate, write the Service Delivery Plan (if requested), review the Service Delivery Plan, and provide ongoing support.

 

MRCI is proud to offer both Consultation Services and CFSS. In compliance with regulations set by DHS, we can only provide one of these services to an individual.

Your MRCI Consultation Services representative will be available to meet virtually, in person, or on the phone based on the participants choice.

How to sign up for Consultation Services

Contact your Lead Agency or County

Determine program eligibility by Finalizing Medical Assistance (MA) Application, and completing necessary assessments.

Select MRCI CDS as your Consultation Services Provider

Inform your Lead Agency, Case Manager, or Assessor you've chosen MRCI CDS as your Consultation Services Provider.

Contact Us to Schedule your personalized intake meeting

Service Delivery Plan is written with your Consultation Services provider.

Starting your services

Once the Lead Agency approves the Service Delivery Plan, the Lead Agency, Case Manager, or Assessor will send the plan to the provider of your choice for CFSS.
